Author's note: You will notice that for certain enemies you will see a tilde (~) with the number of appearances. This is because there are many different number of appearances you can have depending on if you include cameos, flashbacks, etc. If a tilde appears, this is meant to give you an approximate number, not to try and give the exact number of appearances.
Daleks

Home Planet: Skaro
The Daleks are the most evil race the galaxy has ever produced. They are the Doctor’s most consistent and dangerous enemy with their one and only goal being to “liberate” the universe from anything that is non-Dalek.
Originally, they were humanoid creatures called the Kaleds, who were peaceful and sophisticated. However, after a terrible chemical war with the planet’s other race, the Thals, they became terribly mutated. They withdrew to an underground metallic city, created metal casings to protect themselves, and became the Daleks. They had no contact with the outside world until five hundred years later when they imprisoned a small group of travelers. One of these travelers was the Doctor.
The Daleks, perhaps because of their long isolation, believe themselves to be the greatest power in the universe. They believe they are ultimately invincible, and they see it as their responsibility to exterminate inferior species. They achieve this goal by enslaving the inhabitants of planets, utilizing the planet's resources to build a more efficient army, and then moving on to the next world. They attempted to use this method on Earth in the 22nd century, but were repulsed by the Doctor with the help of freedom fighters.
After these events, the Daleks nearly destroyed themselves in a time war with the Time Lords. Very few survived, and it remains to be seen whether they will be able to reclaim their empire that once spanned across 70 planetary systems.

The Meddling Monk

He first encounters the Doctor in 11th century Northumbria where he is attempting to prevent William the Conqueror from winning the Battle of Hastings. In order to avoid suspicion from the natives, he sets up shop in an abandoned monastery and takes on the guise of a monk. While these monsters might appear to be your average fluffy Abominable Snowman, they are in fact anything but. These incredibly dangerous (and cute) Yetis are actually the creations of The Great Intelligence, a formless entity from a mysterious dimension. The Yeti are robot servants of The Great Intelligence and are powered by a small spherical ball underneath their fur. Although they lack swiftness, they are virtually impossible to stop due to their strength, size, and fear factor. The Second Doctor, along with the help of Professor Travers, prevented the Yeti and The Great Intelligence from conquering the Earth on two separate occasions.
Interesting fact: The Yeti’s roar is actually the sound of a flushing toilet slowed down.

Although they are a war-like race, they have not been able to advance their conquests much further than their own planet, perhaps because of their aversion to warm climates. Their lone major invasion of Earth went astray when the Second Doctor managed to send their invasion fleet whirling into the sun in The Seeds of Death.
Eventually, the Ice Warriors are able to curb their aggressive nature, and since then they have become well respected members of The Galactic Federation. They have vowed to renounce violence, except in self defense, and mostly act now as a Federation peacekeeping force.

It can enter and manipulate any type of plastic, often taking the form of mannequin-looking beings. They conceal dangerous weapons within their hand, but their movement is limited, and they use no verbal communication. The Nestenes arrived on Earth in 1970 in hollow plastic meteorites, where they proceeded to replace government officials with human-looking Autons. This attack was thwarted by the Third Doctor and UNIT.

The Silurians and their cousins the Sea Devils are part of an ancient reptilian race that once ruled Earth before the dawn of mankind. Long ago, their scientists predicted that a geological catastrophe would befall Earth, destroying all life on the surface. In order to survive, they went underground, built hibernation chambers, and slept for millions of years. However, the disaster they anticipated never came, and when they awoke they found man as the dominant species on Earth. The first Silurians awoke in 1970 and several others bases have awakened since then. In our first few dealings with them, they have been hostile, believing themselves to be the rightful owners of Earth.
The only reason there has not been full-scale war is that the majority of the Silurians have not awoken from their hibernation chambers due to a flaw in the deactivation circuits. Although there have been negotiations several times between the few awakened Silurians, they have always gone awry, resulting in continued conflict. With limited man -power and equipment, the reptiles have been overwhelmed, and then the events quickly hushed up by military agencies. Despite these minor victories, there are still hundreds of hibernation bases and millions of Silurians underneath the Earth and its seas.

The Master has intelligence levels similar to that of the Doctor, where they studied together on Gallifrey, and he has the ability to hypnotize and manipulate people. He can disguise himself and blend in to avoid detection, and he prefers stealth to brute force. He is coldly calculating and a complete sociopath. A fellow Time Lord once told him, “You are one of the most evil and corrupt beings this Time Lord race has ever produced. Your crimes are without number and your villainy without end.”
In Last of the Time Lords, the Master is shot and presumably killed, and he has not reappeared in the series since. However, the Master has been thought to be dead before...

Omega

Home Planet: Gallifrey
Omega is one of the founding fathers of the Time Lords and is a legendary figure in Time Lord history, being equal in importance to Rassilon. As the legend goes, in an attempt to create a power source to fuel his time travel experiments, Omega accidentally started a supernova, which collapsed into a black hole. While Rassilon survived and was able to capture the nucleus of the black hole and harness its power to perfect time travel technology, Omega was engulfed in the hole and presumably killed.
However, Omega did not die, but rather he became stranded in an antimatter universe that the black hole spawned. Due to the strange properties of this universe, he found he could shape the world to his will, but he could not leave. Although he built special armor and a mask to protect himself from the radiation, over millenia his body wore out and disintegrated; he exists now only because his will insists he exists. Due to years of isolation, he has turned insane, and he wishes to punish the Time Lords for "abandoning" him. He is determined to escape the world of antimatter, and exact revenge on those responsible for his exile.

Sontarans

Home Planet: Sontar
The Sontarans are a race of humanoids who are completely devoted to warfare. They originate from the planet Sontar which is nicknamed “the southern spiral arm of the galaxy”. It is best known for its strong gravitational field.
Sontarans have an unusual brownish colored skin and a distinctive dome-shaped head. They tend to be shorter than most humanoid species, but their low center of gravity actually makes them more formidable since it is harder to knock them over. They do however have one weakness on the back of their neck called the probic vent, which is used for nutritional and cloning processes. Although Sontaran generals proudly claim that it provides incentive to continue to advance upon their enemies (since retreating would leave the area exposed), it does create a simple and obvious way of putting them out of action. Even minimal contact will leave them incapacitated for short periods of time. Their main enemies are the Rutans, whom they have been fighting a galactic war with for millennia. Neither side so far has been able to hold any type of lasting advantage over the other.
Despite these weaknesses and setbacks, the Sontarans are one of the most formidable species in the galaxy.

Davros is the scientist who designed and created the Daleks. During the war between the Thalls and Kaleds, he was the head of the Kaled's Elite Scientific Division, a military department created to invent new technologies to help win the war. In an attempt to reuse mutated Kaled soldiers, Davros created tank-like machines for them to move about in. Through genetic experimentation, he removed their emotions that he perceived as weaknesses like compassion, mercy, and kindness. He christened them the Daleks, an anagram of the name of his own people. As his experiments progressed, Davros became obsessed with his creations, believing them to be superior to all other forms of life.
Although it is never explained how Davros came to be in his present physical condition, it seems reasonable to assume that his crippled body is the result of the chemical warfare on Skaro. He cannot use his left hand and with both his eyes welded shut, he sees only through a cybernetic eye on his forehead. His wheelchair, which encloses the lower half of his body, (an eventual inspiration for his Dalek design) gives him not only mobility, but it also contains his life support systems. If he is removed from the chair, he will die.
Davros is a complete megalomaniac. Apart from giving up his own life, he will do anything to ensure that the Daleks are successful in their mission to rule the galaxy.

The Black Guardian is the anthropomorphic personification of chaos and entropy, and is one of the six Guardians of Time; six transcendental beings entrusted with various aspects of the universe. His direct counterpart is the White Guardian, the overseer of light and order. Guardians never directly interfere with the universe themselves, but instead use human agents to carry out their wishes and desires.
The Black Guardian has hated and wished to destroy the Doctor ever since the Doctor outsmarted him and recovered The Key to Time on the behalf of the White Guardian. During the Doctor's Fifth incarnation, the Black Guardian sent a Trion named Turlough to kill him, but the plot failed when Turlough refused to carry out his mission and chose to side with the Doctor instead. Since this attempt the Black Guardian has not showed his hand again, but the Doctor has been warned that he is sure to return, and that he will now be more determined than ever to exact revenge.

The Mara
Appearances/First Appearance: 2 (Kinda)
Home Planet: Manussa
The Mara is a psychological parasite created by the greed and hate of the people of Manussa via the power of the Great Crystal. When it chooses to have a corporeal presence, it takes on the form of a giant snake, but most of the time it lives inside of a host, feasting on negative emotions festering in the host's psyche. Once under the Mara's control, its victims have almost no free will of their own; their teeth will begin to redden and the symbol of a snake will appear on their forearm. According to legend, long ago the Manussans defeated the Mara and exiled it to the "dark places beyond".
The Doctor first encounters the Mara on the planet Deva Loka where it possesses Tegan after appearing to her in a dream. Though the Doctor is able to break the beast's grip on her and believes it to be dead, it has in fact only retreated deep into Tegan's psyche. It resurfaces in Snakedance, but the Doctor defeats it again with the help of the sage Dojjen. This time, the Doctor proclaims the Mara to be truly dead, and the monster has not reappeared since.

When it chooses to have a corporeal presence, it takes on the form of a giant snake, but most of the time it lives inside of a host, feasting on negative emotions festering in the host's psyche. Once under the Mara's control, its victims have almost no free will of their own; their teeth will begin to redden and the symbol of a snake will appear on their forearm. According to legend, long ago the Manussans defeated the Mara and exiled it to the "dark places beyond".
The Doctor first encounters the Mara on the planet Deva Loka where it possesses Tegan after appearing to her in a dream. Though the Doctor is able to break the beast's grip on her and believes it to be dead, it has in fact only retreated deep into Tegan's psyche. It resurfaces in Snakedance, but the Doctor defeats it again with the help of the sage Dojjen. This time, the Doctor proclaims the Mara to be truly dead, and the monster has not reappeared since.

Sil

Home Planet: Thoros Beta
Sil is a slug-like creature who works for the Galatron Mining Corporation on Thoros Beta. Sil is devoid of any moral scruples, and he enjoys exploiting desperate parties into giving him the lowest possible price for their mineral rights. He has a grotesque fascination with both violence and disfigurement as can be seen when he tries to use Peri as a test subject for a scientific experiment that mutates the subject's cells.
The Doctor first meets Sil on the planet Varos where he is trying to force the people into giving him an exorbitantly low price for the precious mineral Zeiton-7. He also makes an appearance in the second part of The Trial of Time Lord, where he is killed by King Yrcanos, but since the Matrix that showed this had been tampered with by the Valeyard, it is impossible to say whether this actually transpired.
Sil is widely regarded as one of the most memorable monsters the show has ever created. Particularly effective is Sil's sibilant voice and ululating laugh, both of which were devised by the actor portraying the character, Nabil Shaban.

The Rani, Master, and the Doctor all knew each other at a young age (The Doctor and the Rani are the exact same age) and possibly attended academy together. The Rani was exiled from Gallifrey after a gigantic rat she created through genetic experimentation ate the Lord President's cat and then bit the President himself, triggering a regeneration.
The Rani is a scientific genius who puts everything, including moral principle, aside for the sake of her research, which usually involves the tampering with of some poor specie's biochemistry. She cares nothing in particular for power, unlike the Master, whom she finds overtly dramatic and somewhat stupid but only for a steady supply of test subjects and a place to conduct her experiments undisturbed. In Mark of the Rani it is hinted that she enslaved a whole world as a means of conducting her research until the people became violent as a result of her experimentation and forced her to flee the planet.
Interesting fact: the word "Rani" means "queen" in the Hindu language.
